Windows Instructions for using Fillable Forms
Click View Form and then DOWNLOAD the form to your computer. DO NOT SKIP THIS STEP. Data may be lost if you do not DOWNLOAD the form before entering data. To DOWNLOAD, use the icon at the top right of your screen or right click and choose Save As.

Take note of where the file is saved on your computer. Now open this file from your computer using Adobe Reader.  If you need to download a free copy of Adobe Reader, click here .

1. On the form, fill in required fields.
2. Click "Save to my computer" and/or "Print" to keep a copy for your records.
3. Click "Submit for Review" to email the form. Enter email address(es) for current Chair(s) as identified at the front of the CSEII Directory.

(You may also deliver a printed copy of the completed form to the street address of the Chair(s).)
